the claimant’s identity, living/deceased status, receipt of benefits
checks, activity level and employment status. This service is
recommended on all cases involving extended disability.

A claimant interview verifies Social Security Number, current address
and telephone number, and a health status. A description of the
claimant’s residence and lifestyle is provided.

With the claimant’s consent, a photograph of the claimant is taken at the
time of the interview. In instances where claimant contact is not
made, a photograph of the residence is provided.

Claimant Interviews are completed via scheduled appointment or unannounced
visit at our client’s direction.

Capital Service Bureau, Inc. will maintain the case on diary to make
periodic visits as requested. |
